Breast Augmentation Revision: Patient 35

This patient came to see me after another plastic surgeon left her with a result that she was not pleased with.  She had ultra high profile 380cc Mentor breast implants placed in an above the muscle location and did not like the wide appearance of her breasts or the bottoming out (inferior displacement) of the implants.  She is shown after revision with highly cohesive SCF 650 Allergan Natrelle Gummy Bear Silicone breast implants, pocket modifications and internal bra support with galaflex.  Using mesh in the breast is an off label technique but often used by plastic surgeons to give extra support when there is not enough natural tissue support.  The profile was also switched from ultra high to full profile.  I believe that very commonly the wrong profile of breast implant is elected either by the surgeon or by the patient for their surgeries and has large implications.  Often the patient (and I suspect unfortunately the surgeon) does not really understand what different profiles actually mean or deliver.  An ultra profile implant gives tons of projection but has a very small diameter and thus it is neither wide nor tall.  This is not going to give much upper pole or cleavage with a smaller 380cc implant in a patient with a wide chest.  Rather she needs an implant wide a wider diameter to achieve these goals.   She is shown before and after with a significantly improved result.
