“En Bloc” Breast Implant Removal

Dr. Sorokin offers explantation of implants along with the capsule surrounding the implant. Sometimes the implant can be removed with the entire implant capsule, often described as an “En Bloc” removal. There are websites on the internet that state this is the only way implants should be removed and if a surgeon does not use this technique they are not doing a proper removal. This is inappropriate advice as sometimes a capsule is thin or non-existent, and sometimes it is adherent to structures that should not be damaged. In fact, in Spring of 2022 The Aesthetic Society released data to plastic surgeons stating that preliminary studies show no improvement benefit in different types of capsulectomies. Since an En Bloc type of capsulectomy can be more deforming to breast appearance, this is something that needs to be discussed on an individual basis. The plastic surgeon should do an appropriate procedure based on what will be the best, safe treatment for each patient.

There are a couple basic options for breast implant removal: we can remove and replace, or simply remove.

Revision Surgery

While the typical lament is “I wish I had gone bigger,” there are also women who choose to go smaller. An explantation with a subsequent implantation is usually called a breast implant revision surgery or secondary breast augmentation.

Post-Removal Breast Lift

Depending on the size of the implants, their age, and the age of the patient, the patient’s breast skin may have lost its ability to “snap back” to a pre-implant firmness and contour. In these cases, a breast lift, or mastopexy, may be recommended after implant removal. This is usually done in staged procedures in order to optimize the appearance of the explanted breast.

Breast Implant Removal and Contour Shaping with Fat Transfer

Some patients may benefit from contour with fat transfer after implant removal. Fat obtained from liposuction can be strategically placed to shape and add volume for the skin to contract and reshape around.
