This 36 year old lady was unhappy with the size and shape of her implants.  She also does not like the saline feel.  Her right breast is a little low and they are too widely set.  We discussed revision.  I usually switch ladies to silicone when they do not like how they feel however she wanted to use the "Ideal Implant" which is a type of saline implant that has multiple interior lumens and baffles that keep the saline from moving around.  This is designed in order to minimize rippling and create an implant that feels more like a silicone implant without the worry of silicone silent ruptures.  She is shown before and after pocket modifications as well as increasing her size from 325 go 450cc.   I have seen her postoperative and her postoperative exam does have less rippling than she did prior to surgery.  She is happy.  I do not feel that they feel the same as silicone however.  I still feel the edges of the implant and it has more of the feel of saline than silicone.  My feeling after doing several of these Ideal Implants and seeing them in follow up is that they are nicer implants than regular saline but not as nice in performance as silicone.  Is that worth the extra money of the device?  To some ladies yes.  This implant makes a great extra option to discuss with ladies afraid of silicone.
