So many of my patients come in looking for a modest, conservative breast enhancement. They tell me things like "I'm not looking to be a porn star" and that they "want to fill out my bra." This is a classic case of a mother of 3 who lost volume after pregnancy looking to refill her breast volume but not be too big or too fake. This type of patient often presents with a little bit of sag to her breast tissue and is on the borderline as to whether or not she could benefit from a breast lift. The downside of a lift is that it leaves permanent scars as a tradeoff for the lift that it provides. For a woman who is looking for a very round "fake" result postoperatively just going with an implant in a case like this will not give super round fake breasts. Instead it will give a very natural appearance like the one shown here. I believe that determination of whether or not to lift in a borderline situation must be made based on what the patient is looking for. If she will be happy with a naturally shaped full breast like the ones shown then avoid the lift. If she wants to eliminate all sag and have round upper pole fullness she needs to have the lift. She is shown preoperatively and postoperatively at about 6 weeks as a small C cup.
