This bodybuilder came to see me concerned with the look and feel of her older saline breast implants. She had rippling and could feel the shell of her saline implants. In addition she felt they had bottomed out over time and were too low. In ladies with very low body fat and very small natural breast tissue saline implants often feel much less natural than silicone. While silicone gel implants can still have some rippling they do so to a much lesser degree than saline does. This patient is shown before and after breast enhancement revision with internal bra pocket modifications as well as change to larger silicone implants. I was able to reuse her periareolar scar and not make a second incision on her breasts. Truth be told I do not like the periareolar incision as it leaves a scar on the front of the breast, has a higher infection rate, and can affect nipple sensation to a much larger degree than other incisions. That being said when a patient comes in and a surgeon has previously used this incision I try to use it if possible.
