This patient has had a very complicated history with multiple breast surgeries starting about 25 years ago. Initially she had saline breast implants placed via armpit incisions but within a year needed a revision through the crease incision. Fast forward 20 years she had some drooping to the breasts and went to see a plastic surgeon near the Jersey Shore who removed the implants and did a lift. She received an anchor mastopexy but was unhappy that her nipples were too high and that she had minimal upper pole fullness nor firmness to the breasts. She came to see me and we decided to place 415cc silicone implants and to remove tissue from the bottom portion of the breast in order to pull the areolae down. This is a problem that is difficult to fix. She is very happy with her new appearance postoperatively.
