A breast lift will always make breasts a bit smaller unless an implant is used to maintain or increase volume. Tightening up the existing breast tissue as well as removing excess stretched out skin will make the breast smaller. If the lady doesn't mind being a bit smaller than a breastlift without implants can be done. If the lady has lots of breast tissue to begin with, avoiding the breast implant may be possible. A lift can often be performed at the same time as an implant but not always. When a breast is very significantly saggy and needs to be elevated a long distance it becomes unsafe to place the implant at the same time and often the procedures are performed in two staged separate surgeries. This decision is based on many factors and different plastic surgeons have different opinions on when best to combine the procedures. I base this decision and recommendation on clinical examination of existing tissue quality and position more than anything. Regardless, this patient is shown before and after breast lift vertical mastopexy lollipop type with a small 235cc silicone gel breast implant.
