Sometimes despite going to a well respected board certified surgeon results are not what we want them to be. This young lady had surgery by another surgeon originally through an armpit (axillary) incision with saline implants. Unfortunately once everything healed the left breast was lower than the right breast. This can happen for many reasons. Sometimes it is due to surgical technique but very often this is due to lack of strength of supportive tissue allowing an implant (or both implants) to settle below where they need to be. In this case the left breast was lower than the right and therefore the nipple on this side appears too high. In order to fix this I went in through a crease incision and tightened and supported her pocket with an "internal bra technique." It is very difficult to do revisions (if not impossible) properly through an axillary incision as direct access to the problematic area allows direct control and fixation of the tissues. Thus patients need to know when selecting an initial armpit incision that future revisions may require a new incision. This patient is shown before and after revision with new 500cc high profile silicone gel implants. Her implant position is fixed and has stayed fixed 4 years later when I last saw her. Her rippling is resolved and she has a nice enough result that she has been able to pose for Playboy!
