This is a 40-year-old lady who was very unhappy with the shape of her breasts after previous breast augmentation by another surgeon. She had saline implants placed and over the years noted that her native breast tissue was hanging over the edge of her implants and creating a waterfall or snoopy deformity appearance. She wanted initially to simply change out her implants and I refused to do this as it would not help. Her problem is her natural tissue is not lifted by her already large (almost 500cc) saline implant and she needs skin removal. She is extremely reluctant to any incision as she does not want a scar however after educating her and explaining why a lift is necessary she decided to proceed. Her incision was kept limited to just around the areola and she is shown before and several months after surgery.
