This is a case of a 33-year-old lady who desired breast augmentation. She wanted to receive around 500cc implants and was upset with the shape of her current breasts. She also was an avid exerciser and wanted her implants above the muscle so that there would be limited movement with muscle flexing and exercising. In addition given the mild amount of breast sagging that she started with this position would give her more lift. She is shown before and after placement of 505cc high profile silicone gel implants. Many women think that implants only should be put below the muscle or that somehow submuscular is "better" than subglandular or above the muscle. There are actually very specific, very clear advantages and disadvantages of different pockets and the proper pocket for a given patient should be something individualized per patient. While I usually go under the muscle, I certainly do not always put implants under the muscle depending on each specific case. There are many outstanding breast surgeons who believe this is a change in thinking that plastic surgeons and patients should start to understand.
