This is a patient of mine who came in looking to be proportional and full. She was an A cup and wanted to be a proportional C. We discussed pros and cons of saline versus silicone and she chose saline implants as she wanted to know if the implant ever developed a leak. With saline leaks are obvious as the implant deflates and the breast goes back to being smaller. With silicone gel cohesive implants leaks are not obvious and ultrasound or MRI needs to be done every 2-3 years to screen for leaks. This is time consuming and can be expensive. Saline is easier but the downside is that especially on lean patients with little starting amounts of breast tissue they will ripple more and feel less natural. These days (2024) in my practice I do over 95% silicone implant but a beautiful result can absolutely be achieved with saline implants as shown here at 6 weeks postoperative.
