This patient has a complicated history.   Prior to coming to see me about 10 years ago she had first had an augmentation with saline 325cc breast implants but then decided to go larger to around 550cc breast implants and get a lift.  Years later the breasts had become droopy again and she had come to see me about 10 years ago for a revision of the lift and wanted to address the rippling.   She is a very thin, petite lady around 102 pounds and has very little body fat so without question a large saline implant is going to lead to rippling.   She was offered silicone breast implants but did not want them because she was afraid about safety.  Years ago there was a moratorium on silicone breast implant use in the United States from 1992 through 2006.  We did not have data to say that silicone breast implants were safe and the only option was saline.  The stigma surrounding silicone breast implants remained for years and many patients would choose saline because of the fear of silicone.  Fast forward to December 2025 as I write this and it has been nearly 20 years since silicone breast implants came back to the open market in the United States and most ladies these days get silicone.  Just in sheer numbers since the post approval studies that silicone has proven to be safe.  In fact when we talk about Breast Implant Illness (BII) there are actually statistically more patients with saline breast implants who have this condition that silicone breast implant patients.  Regardless this patient did not want silicone so she chose the Ideal Implant, which was a saline implant with multiple chambers that was prone to leaking at a much higher rate than regular saline implants.  I have seen this unfortunately in my practice with a large percentage of patients who received them having one leak just a few years later.  I never liked them anyway as while they were advertised as a substitute for silicone because of their unique chambers which minimized fluid movement this supposedly caused less rippling than standard saline.  I never found this to be the case and never really liked them.  Back to this patient, she had a leak now about 7 years out from saline Ideal Implant augmentation and now decided that she would switch to silicone gel.  Motiva breast implants note a 1% leakage rate in their data so hopefully she will have better success with these.  I revised her lift and she is shown after augmentation mastopexy revision with 625cc full profile submuscular anchor mastopexy breast augmentation.   She is much happier with the way these implants feel compared to any of her previous augmentations.
