This 30-year-old patient came to see me for possible breast enhancement. After 2 kids she was left with breasts that were deflated and smaller than desired. We discussed in detail the differences between saline and silicone implants. There are many differences between the two types of implants. I tell all of my patients during consultation that there are three differences that they need to consider. One is the difference in how the implants perform, in terms of look and feel of the implants. Silicone implants are softer feeling and more natural feeling to the touch. Saline implants tend to ripple more than silicone does. The second issue that must be considered is the safety stigma surrounding silicone breast implants. Silicone was not always felt to be safe. Years of research led to the National Institutes of Health (NIH) and the Food and Drug Administration (FDA) to allow them back onto the American market. Despite all of the safety data, there is a history behind silicone and a stigma that persists. Some people are ok with the safety data; others are not comfortable. Certainly I feel they are safe with the voluminous safety data. The third major difference and consideration is long-term detection of leaks with implants. Any implant is a man-made device and can leak with time. With saline, it is immediately obvious as the saline absorbs. With silicone, MRI is required every 2 years.
