This is a 59 year old patient who came to see me with saline breast implants that had been placed 25 years ago in a subglandular via umbilical (belly button) incisions.  What is old seems to be new again as many years ago transumbilical breast augmentation was commonly performed.  Using an incision in the belly button an introducer with a balloon on its end was tunneled up to the breast.  The pocket was developed by inflating a balloon and then the balloon was removed and the saline breast implant was placed in the pocket and filled with saline.  Years later we are now seeing surgeons advocating PreserveTM breast augmentation using again a balloon dissection technique to place implants but now with silicone implants and not through the belly button.  Surgeons migrated away from these remote balloon assisted techniques as I was taught during residency training about the advantages of precise controlled dissection and that blunt was bad.  We shall see how this story plays out over time as social media is emphasizing the great new technique and I question to myself is this really new and is this really an advance?  Regardless here we have a patient 25 years out who has developed capsular contracture grade 4 with hard, tight painful breasts.  A capsule is the scar tissue in the pocket that hardens, thickens and becomes tight and constricting.  This patient had developed two tight capsules that during surgery were calcified, thick scarred pockets.  The breast implant revision was performed and pockets were removed and replaced with a neopocket location and new Motiva RSD round 245cc breast implants were placed in the neopockets.  She is delighted at 6 weeks that she feels soft breasts now that do not hurt when someone hugs her and she is able to lie on her belly without pain.
