This is a 25 year old lady who is upset with the size and position of her breast implants.  She had them done by a surgeon in the Philadelphia suburbs 4 years ago but does not like them.  They feel too lateral and low and are not large enough for her liking.  She complains of bottoming out (inferior displacement.). When implants are too low the nipple sits too high and this can be a major problem with wardrobe malfunctions in bathing suits!   We discussed revision with movement of the implants from submuscular to subfascial to get her more central cleavage, larger implants and change from 455cc moderate profile to a 625cc full (high) profile Motiva breast implant.  I also used Galaflex as an internal bra for extra support.  Galaflex is off-label for breast augmentation surgery but is often used when extra soft tissue support is needed.
