This is a 41 year old who originally had breast augmentation by a plastic surgeon in Philadelphia 20 years ago.  She had saline breast implants and woke up one day with a deflated right breast implant.  You can see in her photos the asymmetry of the breasts but also the hard folded up implant shell is poking out of the side in the lateral views.  Saline breast implants are nice in that they literally show you that they have a leak when it occurs.  Silicone breast implants on the other hand require ultrasound or MRI exams in order to image the breast implants to monitor them for leaks.  While this is indeed an advantage of saline they do not look as natural nor do they feel as natural.  Thus most ladies these days opt for a silicone implant and just understand that they need to do imaging every few years. This patient wanted to be slightly smaller and switch to silicone so that they would feel more natural.   She had 425cc saline implants and switched to Sientra 385 moderate profile breast implants.  She is shown before and after 6 weeks her silicone breast implant revision for deflation.
